The owner of an Ohio company has been penalized in connection with a fatal accident two years ago that resulted in a workplace death in Mars.
According to the U.S. Department of Labor, an investigation by the Occupational Safety and Health Administration resulted in a settlement with Industrial Insulation and Coatings LLC.
This settlement is in regards to an incident in June of 2020 in which workers were installing metal shingles on a steep pitched roof at an indoor horse-riding facility in Mars. One of the workers lost his balance and fell causing three other workers to fall from a scaffold. None of the workers were wearing required fall protection and one man died.
The settlement involves accepting a citation for willful violation of construction industry fall protection standard along with a penalty of just over $84,000. Company employees will also be required to complete OSHA training. John Bylar Construction of Spartansburg, PA was also issued citations and penalties in connection with this incident.
